VOLUNTARY LIFE INSURANCE & AD&D

While Jokake provides $25,000 of life insurance, you may wish to purchase additional coverage. With Voluntary Life Insurance provided by Unum, you are responsible for paying the full cost of coverage through weekly payroll deductions. You can purchase this insurance for yourself in increments of $10,000 up to $500,000. Guarantee issue of $100,000 (no medical questions or evidence of insurability required) is available if coverage is purchased when first eligible. If you have purchased employee coverage, you may purchase spousal and dependent coverage. Spousal coverage can be purchased in increments of $5,000 to a maximum of $500,000. Guarantee issue is $15,000 if coverage is purchased when first eligible. The amount may not be more than 100% of the employee amount. Dependent coverage is available in increments of $2,000 to a maximum $10,000 for dependents up to 19 years (or 26 years if they are a full-time student). The maximum benefit for children from birth to 6 months is $1,000. Learn more.
